1) Health & Medical Conditions
I understand it is my responsibility to inform my massage therapist of any current or past medical conditions, injuries, surgeries, allergies, pregnancy, medications, or other health concerns that may affect my session. I agree to update No More Knots Massage of any changes to my health before each appointment.
2) Consent to Touch & Scope of Practice
I understand massage therapy involves physical touch for the purpose of relaxation and/or relief of muscle tension. I give my consent to receive massage therapy services and to the therapist using professional techniques within the scope of massage therapy.
I understand massage therapy is not a substitute for medical care, diagnosis, or treatment, and I have been advised to consult a physician or other qualified healthcare provider for any medical concerns.
3) Draping & Privacy
I understand proper draping will be used at all times. Only the area being worked on will be uncovered, and I may request adjustments to draping or request that any area not be worked on at any time.
4) Minors
If the client is under 18, a parent/legal guardian must provide consent and complete another waiver.
5) Cancellation / No-Show / Refund Policy
I understand and agree to the following policies:
More than 72 hours’ notice: 50% of the service price will be refunded (a 50% cancellation fee applies).
72 hours or less notice (including less than 24 hours): no refund (may be charged 100% of the service fee).
No-shows/Unreachable: treated as 72 hours or less notice.
Event bookings: A non-refundable deposit equal to 50% of the total service price is required to reserve the date/time. The deposit is applied toward the total balance. If the event is cancelled for any reason, the deposit will not be refunded. The remaining 50% balance is due the week before the event.
6) Acknowledgment & Release
I acknowledge that massage therapy may involve risks (including temporary soreness, redness, bruising, and discomfort). I agree to communicate any discomfort during the session so adjustments can be made. To the extent permitted by law, I release and hold harmless No More Knots Massage and its therapists from liability for any injury or adverse outcome related to my massage session, except in cases of gross negligence or willful misconduct.
